Create a form that submits to "http://www.super-search-uk.co.uk/search/" using the GET method.
Create an input box named "search_string".
Create a submit button named "Submit".
If you'd like a Lucky feeling button, create a submit button called "lucky" with value "Lucky feeling?".

And then your all set.
